Achilles tendon pain, also known as Achilles tendinopathy, is a common condition that causes disability and pain in the lower leg. It is frequently encountered in clinics and is characterised by pain and swelling around the Achilles tendon, which connects the calf muscles to the heel bone. While the exact causes and mechanisms of Achilles tendinopathy are not fully understood, several intrinsic factors have been identified.

Tendon vascularity refers to the blood supply to the tendon. Reduced blood flow to the Achilles tendon may impair its ability to repair and recover from injuries, making it more susceptible to pain and dysfunction. Age is also considered an intrinsic factor, as tendons tend to become less flexible and more prone to injury.

Dysfunction in the gastrocnemius and soleus muscles, which make up the calf muscles, can contribute to Achilles tendon pain. Imbalances or weaknesses in these muscles can place excessive stress on the tendon, leading to pain and inflammation.

Other intrinsic factors include sex, body weight, pes cavus foot type (high-arched foot), and lateral ankle instability. These factors can influence the biomechanics and load distribution of the Achilles tendon, increasing the risk of developing tendinopathy.

Achilles tendinopathy is commonly associated with overuse, particularly in activities that involve repetitive jumping, running, or sudden accelerations. However, it is not limited to athletes and can also affect middle-aged, overweight, and sedentary individuals.

Treatment for Achilles tendon pain

Treatment for Achilles tendon pain is often individualised, considering various factors such as the severity of the condition, the individual’s strength and fitness levels, and their specific goals.

Conservative management options may include physical therapy exercises to improve strength and flexibility and orthotics or shoe modifications to correct foot alignment. More advanced treatments, such as extracorporeal shockwave therapy, may be considered in some cases.

When tailoring a treatment program for Achilles tendon pain, healthcare professionals will assess the individual’s overall strength and fitness levels to develop a plan that addresses the condition’s underlying causes and promotes optimal healing and recovery.

Rehabilitation exercises typically focus on strengthening the calf muscles, improving flexibility, and gradually reintroducing activities to prevent further injury. Individuals must follow their prescribed treatment plan and actively participate in rehabilitation to achieve the best possible outcomes.
